TERMS OF THE SWAT MONEY BACK GUARANTEE

~This offer is good for new clients, and is only valid one time per client.

~To qualify for the 30-day money-back guarantee, you agree to follow program guidelines, and put forth your best effort for the full 30 days of the program. If after 30 days, you're not happy with your leaner, stronger and sexier self, we'll refund your payment with no hard feelings! To qualify for the money-back guarantee, here are the guidelines you must follow:

• ~You must attendat least 4 days per week for at least one month and follow the recommended nutrition guidelines. You agree to submit a detailed weekly nutrition journal to your trainer and give 100% effort to your program for 30 days. If after completing the 30-day Trial Period, you have followed all guidelines and are not leaner, stronger and more fit, we'll refund your money!
• ~To request a refund, you must submt a written request to: Jana Holland / c/o SWAT Fitness / 6127 N La Cholla Blvd / Tucson, AZ 85741 within 15 days of. After receipt of request, an exit interview will be scheduled to gather feedback about how we can improve our program.

SWAT BOOT CAMP PROGRAM DETAILS:

~You can reach us at any time. You're our priority, and we want you to have a GREAT experience with SWAT. We'll work with you if and whenever possible, and pledge to handle all transactions and relationships with honesty and integrity.

~Programs include:

SWAT Month to Month Unlimited Boot Camp. Good in one location and time slot only. First month 247, then 199/month with a 30 day written cancellation policy.

SWAT Body for Life includes unlimited boot camp at 149/mo for 12 months minimum. It is a minimum 12 month commitment.

SWAT INNER CIRCLE program includes unlimited training of any kind, at any SWAT fitness studio or boot camp location, and any time slot! 199/month for 12 months. This is a minimum 12 month commitment.

~First payment is due at time of registration. Subsequent monthly payments are automatically billed by EFT once a month, either on the 3rd or 17th of each month until minimum commitment is met and/or 30 days after written cancellation is received.

I understand that activities involved in the fitness program carry with them the potential of harm to myself, my property, loss or damage to my possessions, bodily injury and death. I hereby certify that I understand and accept all risks associated with my participation in SWAT Fitness Boot Camp and agree to hold harmless its management, staff, second parties, or associated person(s) and/or entities from and against any and all liability for any harm to myself or my property that may arise from my participation in this program.

I understand that I must be aware of my physical conditions/limits and that there are risks associated with fitness training activities. Also, I understand that I am encouraged to get a regular medical examination prior to participating in the SWAT Boot Camp program.

I understand that under Arizona law, I have the right to cancel my registration for the program by providing written notice within 3 days of registration.

By registering for the SWAT Boot Camp program, I authorize SWAT Strength Wellness Athletic Training to charge my credit card for the SWAT Boot Camp Program, plus any applicable city service taxes. I understand that my registration is final, and there will not be refunds.

I understand that there are no refunds unless indicated by SWAT's special program guarantee based on the program selected.

I understand that there may be photos / video footage taken that may be used for marketing / advertising and that any media, including video, photos, testimonials (used in full or in part), may be used for promotional purposes, without expectation of compensation for such.

I certify that I have read and fully understand the waiver, liability, terms, and policies and fully agree to abide by the conditions and terms stated above.

You must provide at least 30 days cancellation notice in writing to avoid continued billing. If you are on a 12 month plan, then after the 12 month minimum term, you will continue to be automatically billed (and your rate will be guaranteed) until 30 days after written notice is received.

To CANCEL:
Submit in writing to: SWAT Fitness c/o Ron Holland / 6127 N la Cholla Blvd / Tucson AZ / 85741.
Once you cancel the auto registration, if and when you decide to start back up - you will have to re-registerer at the going rate, and may be required to pay the orientation / registration fee. Please call us to verify that we received your cancellation. Billing will be discontinued 30 days after receipt of cancellation notice.

~Recurring billing is valid for the person who enrolled and is non-transferable to other individuals

~No additional credits or discounts are allowed

~If on a 12 month plan, your monthly rate is guaranteed for 12 months and your rate will not increase during this time! (Account must remain in good standing without disruption in billing)

~Classes must retain a minimum number of participants. If for some reason we have to cancel a particular location or time, you will be offered a Credit toward another program or service of your choice, or a refund. It's not your fault if we have to cancel; so you won't be penalized in any way for it.

~If anything medical is keeping you from participating at your best -see a Doctor, and PLEASE TELL US IMMEDIATELY!

~In the case of a medical issue you may have the option to FREEZE your account for a minimum of one month, and a maximum of 3 months, with a note from a physician stating you are not able to continue the SWAT Fitness Boot Camp program, with the notion that you will return when the physician sees appropriate. You will not be able to participate in programs during that time, and agree to be charged a $25 per month freeze fee for any months while on FREEZE status, or will be credited for camps billed and missed due to medical leave from the time notice is provided to the time I am able to return. Months on FREEZE status are not credited toward a 12 month boot camp commitment option.
